Rates

Sometimes session lengths vary depending on the type of session. Per session rates are set based on the service provided and the length of the service regardless of how many people are in the session. Please see the fee schedule listed below for details on our current rates:

• $125 for 45 minute session
• $150 for couples therapy session
• $150 for 60 minute session
• $180 for Diagnostic Intake

Insurance

Origins is in network with many major insurance carriers including Blue Cross, United, Optum, IU Health, Encore, Community ProHealth, CareSource and Aetna. We also have therapists who are able to accept most Indiana Medicaid plans. Please call us to verify our network status with your insurance plan. We are also happy to file an out of network claim for any plans we are not in network with but these are sometimes covered at a higher deductible or out of pocket rate for out of network.

Payment

We accept cash, check and all major credit cards as forms of payment. We can also take Health Savings Account and Flexible Spending Account Debit Cards.

Cancellation Policy

If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged a $50 Late Cancellation/No Show Fee.
